I would recommend all. You can get cover for all countries apart from a few exclusive ones (I could imagine Lebanon, Gruznia -Forgot english name- , Nth. Korea and so forth would be on the exclusion list.)
Usually cover works by continent.
Just remember, if you are under the influence of drugs or alcohol, you will not be covered. 
Search up ?Travel insurance? on the web and you should find plenty of providers.
For a 3 month trip for comprehensive cover, you should not be paying more than $400 USD.
Check each company for what they cover and how you may make claims.
Insurance companies are cheeky, the best insurance is your travel partners.
